We have a new vacancy for a Clinical Social Prescribing Link Worker to join our Primary Care Network (PCN) team in Jesmond and Lower Gosforth.
About the Role
This is a new role for our PCN, designed to promote health and prevent ill health for our patient population. We are looking for a social prescribing link worker who is a registered Nurse Associate.
It is a permanent, full-time role employed directly by Jesmond Health Partnership but working across all four practices in our network.

Duties and Development
Predominantly, this role will work with care home patients or others who are housebound as part of our Over 65s teams.
A key duty will be to carry out monitoring and healthcare improvement skills in line with year of care, annual reviews, and personalised care support planning. This will include the ability to safely undertake procedures such as venepuncture, ECG recording, measuring and interpreting blood glucose levels, recording and interpreting vital signs, assessing continence care, bladder and bowel care, pressure area care, basic wound care, nutrition, hydration, foot care, skin integrity, mood identification assessments.
You will also support patients to improve and maintain their mental, physical, behavioural health and well-being, recognising health inequalities and proactively working to reach people from all communities.
There will be opportunities for the successful candidate to both develop their own professional skills and shape and develop the position within our multi-disciplinary team.
About You
You will be a registered Nurse Associate, having completed your foundation degree and hold professional registration with the NMC.
As well as the core competencies listed above, the successful candidate will be an effective communicator with patients, colleagues and other community organisations. You will be able to demonstrate a communication style that can give and receive sensitive information about a patients’ health, while taking into account different levels of understanding, cultural background and other preferences.
